Nomination to Candidacy for the PhD Degree - to be submitted to Graduate Office after the completion of the Prelims (note for EEB both part I and II must be completed). |

Nomination of Research Committee for the PhD - Should be submitted 6 months prior to your defense date but may be submitted earlier if you wish. Submit the form to the Graduate Office for the Chairperson's signature. |

Change of Research Committee - used if you decide to change your committee. |

Transfer of Credit - Upon completion submit form to the Graduate Office for the Chair's signature. |
